In order to optimize resource utilization, Dr. Dipti Sagar, MD has entered into a cost-sharing agreement with Los Angeles Integrative Gastroenterology & Nutrition, Inc. Utilization of resources pursuant to the cost-sharing agreement includes, but is not limited to, workspace, workforce, marketing, website, use of merchant accounts, billing services, electronic medical records, and more. Each party to the cost-sharing agreement is separate and independent of each other and is operated accordingly. Each physician provides opinions and services only to those patients who have signed a Doctor-Patient Agreement with that specific physician. Nothing in a Doctor-Patient Agreement should be construed or interpreted as creating a physician-patient relationship with any party to the cost-sharing agreement other than the Provider. Any questions, issues or concerns arising out of or related to the opinions or services provided by the Provider must be addressed directly with the Provider.
