1. An invitation to our “Patient Portal” will be sent to you.
2. Please create an account on Patient Portal with a Password.
3. On “Home” page Click on “See More” link under your upcoming appointment.
4. Please click on “complete New Patient Medical History”.
5. Complete all the links on the right-hand side:
